Get Referral Help From Our Physician Liaison Team
The easiest way for you to refer a patient to us is to reach out to our Physician Liaison team. These professionals work closely with referring providers in our area to ensure ease of access to our specialty providers and services.

Refer Your Patient to a Baystate Health Provider or Practice
You also can fill out a referral form and fax it to us to start the referral process for your patient. We have a form for pediatric patients and a form for adult patients.
Once you have completed the form, fax it to the fax number found on the referral form. Depending on your preference, we can either make the appointment and fax it back to your office, or we can contact your patient directly to set up their appointment.
