SPECIALIST APPOINTMENT REQUEST (BMP Line)
The BMP Line provides you a centralized referral process where you can refer patients to any of the Baystate Medical Practice Specialists.
Your team provides basic patient information and our scheduling team coordinates the patient booking, including directly contacting the patient and following up with your office.
Complete the Online Intake Form and submit electronically. We will send you a confirmation with the date and time of the appointment or status. The expected response time is 48 hours pending our success in contacting the patient.

BAYSTATE MEDICAL PRACTICE REFERRAL SERVICE
The Baystate Medical Practice Referral Service team can assist you with scheduling new urgent appointments with any of our specialists. Our trained staff is available Monday - Friday from 8a.m. to 4:30p.m. They will help ease the appointment process and provide support for navigating the referral process for complex and acutely ill patients.
BEHAVIORAL HEALTH SERVICE REQUEST
The Central Intake Services intake coordinators will assist with information and recommendations about the appropriate level of care needed concerning:
- mental health and substance abuse services
- insurance verification
- pre-authorization for care.
