
To participate in Baystate Adult Weight Management you must meet Body Mass Index (BMI) criteria. The criteria are:
- BMI over 35 with co-morbidities, or
- BMI over 40 without co-morbidities.
Co-morbidities include diabetes, hypertension, high cholesterol, heart disease, sleep apnea, severe arthritis of the knees or hips, and Polycystic Ovary Syndrome.
You can easily calculate your BMI online.
If you meet the BMI criteria, you must contact your primary care physician for a referral to the practice. This is very important for insurance purposes.
We also recommend that each patient contact their insurance provider to inquire about coverage under their insurance plan for each of our services, such as:
- Nutrition counseling
- Behavioral counseling
- Clinical labs and tests.
Any bills for services received at the practice that are not covered by insurance are the responsibility of the patient.
