Patient Information

Patient Instructions:

Once you have been scheduled for an appointment:ALT

  • Bring all radiological test results with you for the first visit (i.e. films, MRI's, CT scans)
  • Bring a current list of all the medications you are taking
  • Complete the Initial Assessment form (grey questionnaire) that will be mailed to you a week prior to your appointment
  • A nurse will call you at home, to answer any questions, confirm that you received the form, and remind you to bring your medications with you or provide a detailed list of all medications that you are currently taking
  • The standard waiting time is generally 2-4 weeks, although in some instances you may be able to be scheduled within a shorter period of time.


We participate in most insurance programs offered in our area, including Medicare, Medicaid, Health New England, HealthNet, Tufts, HMO Blue, the Aetna Plans, the City of Springfield Plans, Mass Mutual Preferred Plus, Blue Shield and Workman's Compensation. If you have any questions about whether your specific health plan covers our services, please call the toll-free number on the back of your insurance member ID card. Baystate Pain Management Center's charges have two components:

  • Facility charge: this covers charges for your visit and any procedure
  • Provider charge: this covers the fee for the provider which is billed by Springfield Anesthesia Services, Inc.

Should your plan require a co-payment please be prepared to pay for this at the time of your visit.
