Baystate Visiting Nurse Association & Hospice takes care of patients ranging from newborn babies to elderly, mostly in their homes. Some are recovering from illness or surgery while others are coping with the end of life.
To qualify for home health services, a patient must have a skilled need (nursing, physical therapy, or speech therapy) on a part-time, intermittent basis and under physician’s orders. Medicare and some insurance plans require a patient to be homebound.

Our wide range of home health services available to patients includes:
- Skilled nursing care for new diagnoses or existing conditions
- Medication management, including new and IV medications
- Anticoagulation & blood draws
- Rehabilitation therapies: physical, occupational, and speech
- Total Joint Replacement rehabilitation
- Medical equipment assessments
- End-of-life care requiring hospice services for patients of all ages.
(Please note: Medicare does not require hospice patients to be homebound.)
